Services are aligned with the goals outlined in the Individual Support Plan (ISP) and are delivered with consistency, compassion, and a strong emphasis on practical growth and independence.

Our In-Home Services Include:

1. Personal Assistance Services

  • Support with Daily Living Activities (ADLs):
    Assistance with bathing, dressing, toileting, mobility, eating, and transferring.
  • Medication Support:
    Reminders or administration are allowed under provider and state guidelines.
  • Basic Health Monitoring:
    Includes wellness checks, vital sign tracking, and health-related reminders.
  • Meal Assistance:
    Help with preparing meals, feeding, and following dietary restrictions or preferences.
  • Household Maintenance:
    Light housekeeping, laundry, and maintaining a clean, safe living environment.

2. Skill Development and Independent Living Training

  • Personal Care and Hygiene Training:
    Coaching in grooming, dressing, and managing daily routines.
  • Budgeting and Money Skills:
    Hands-on support for handling money, making purchases, and learning basic financial skills.
  • Time and Task Management:
    Building awareness of daily schedules, routines, and organization techniques.
  • Transportation and Travel Skills:
    Learning to navigate the community, use public transport, or travel independently.
  • Home and Personal Safety Training:

3. Behavioral Support Services

  • Implementation of Behavior Support Plans:
    Staff follow individualized behavior strategies developed by clinical teams.
  • Observation and Reporting:
    Documentation of behaviors and progress, shared with the clinical team or coordinator.
  • Crisis Support and De-escalation:
    Staff are trained to manage challenging behaviors in a calm, respectful, and supportive manner.
  • Positive Behavior Reinforcement:
    Encouraging desired behaviors through consistent feedback and support.

4. Community Integration and Social Engagement

  • Support for Communication Needs:
    Assistance using communication devices and encouraging self-expression.
  • Participation in Community Activities:
    Help attending local events, volunteering, or social outings as identified in the ISP.

    Support in developing social skills, initiating conversations, and maintaining friendships.

5. Health & Wellness Support (Per ISP and DMAS Guidelines)

  • Medical Coordination:
    Assistance with scheduling, reminders, and coordinating healthcare appointments.
  • Support During Appointments:
    Staff may attend medical visits to provide support, if approved in the ISP.
  • Assistive Device Training:
    Help with the correct use and maintenance of mobility aids or medical devices.

6. Family and Caregiver Support

  • Training and Coaching:
    Teaching caregivers how to implement support strategies and routines based on the ISP.
  • Short-Term Respite Care:
    Temporary care for individuals to give families a break (when authorized).
  • Progress Communication:
    Regular updates on individual progress, goals, and any changes in needs.
  • Access to Resources:
    Providing educational tools, planning guides, and other caregiver supports.

Service Coordination and Communication

  • Collaboration with Case Managers and Coordinators:
    Participation in team meetings, plan reviews, and interdisciplinary discussions.
  • Comprehensive Documentation:
    Accurate and timely recordkeeping of services, progress, incidents, and compliance reports.
